Clinical Indications

When PEP may be clinically indicated.

PEP is a medical intervention. Your doctor will assess your specific circumstances, but the following are situations in which patients typically seek a PEP consultation.

Condomless sex with a partner of unknown HIV status

Including casual encounters or any situation where a partner's status is uncertain.

Condom broke or slipped

Barrier failure during intercourse, even with a regular partner whose status you are unsure of.

Needle or sharps exposure

Including shared needles, needlestick injury, or contact with blood through an open wound.

How quickly should I come in after a potential exposure?
As soon as possible. PEP is most effective when started within 24 hours, remains clinically indicated up to 72 hours, and is not initiated after 72 hours. If you're unsure whether the exposure qualifies, message us — the doctor will assess it.
Can I get PEP from a pharmacy instead?
No. PEP is a prescription medication that requires a doctor's assessment, a baseline HIV test, and medical supervision for the full 28-day course. This is why a clinic consultation is necessary before any medication is issued.
What actually happens at the consultation?
The doctor takes a brief, medically-focused history, performs a 4th-generation HIV test to establish your baseline status, and assesses whether PEP is clinically indicated. If it is, a 28-day treatment course is prescribed along with scheduled follow-up tests.
Is this kept private? Will my family or employer find out?
Your consultation and records are protected under Malaysian medical confidentiality law. They are not shared with family, employers, or insurers without your explicit written consent.
What is the 4th-generation HIV test?
It is a standard combined antibody-antigen test that establishes your current HIV status before PEP is considered. This is an essential clinical step — PEP is only appropriate for patients who test negative at baseline.
Can PEP be taken during pregnancy or breastfeeding?
Yes. PEP can be prescribed during pregnancy and breastfeeding, but medication selection is individualised. This is discussed fully during the consultation.
What happens after the 28-day course?
Follow-up HIV testing is scheduled at 4–6 weeks and 3 months after exposure to confirm your status. Your doctor may also discuss ongoing preventive options such as pre-exposure prophylaxis (PrEP) if relevant.
Is PEP 100% effective?
PEP is highly effective when started early and the full course is completed correctly, but it is not 100%. Effectiveness depends on timing of initiation, adherence to the full 28-day course, and avoidance of further exposures during treatment.
